目的探讨在非地中海贫血以及轻型β地中海贫血的人群中,缺铁对血红蛋白电泳法测定血红蛋白A2(HbA2)筛查地贫的影响。方法分别分析309例非地中海贫血体检及缺铁人员和92例轻型β地中海贫血患者中HbA2、血清铁的变化趋势和分布特点,讨论缺铁对血红蛋白电泳法测定HbA2指标在轻型地贫及非地贫人群中的影响。结果非地中海贫血人群中缺铁组与非缺铁组的HbA2值之间差异有统计学意义,而轻型β地中海贫血患者中缺铁组与非缺铁组的HbA2值之间无统计学差异;非地中海贫血人群中缺铁组的HbA2和血清铁水平间存在正相关关系,即缺铁的严重程度与HbA2的降低程度平行,而轻型β地中海贫血患者中缺铁组的HbA2值和血清铁水平之间未发现趋势性的关联。结论非地中海贫血人群中缺铁可以导致HbA2水平降低,且HbA2的降低值与缺铁的严重程度有关;而在确诊轻型地中海贫血的人群中,缺铁对HbA2的影响可能被尚未知的因素抵消,而未显示出类似非地贫患者的改变。
Objective To investigate the effect of iron deficiency on the determination of thalassemia by hemoglobin electrophoresis in hemoglobin A2 (HbA2) screening in non-thalassemia and beta-thalassemia minor patients.
