目的:探讨机械通气治疗重症胎粪吸入综合症(MAS)的效果。方法:将38例重症MAS患儿根据血气及胸片结果分为肺不张组与肺气肿组,对两组所用呼吸器参数进行对比分析。结果:38例治愈26例(68.4%),死亡(包括放弃)12例(31.6%);常见合并症为肺炎、气胸、持续肺动脉高压和新生儿呼吸窘迫综合症。结论:根据不同病情选择合适的呼吸机参数及通气模式,积极防治并发症是治疗MAS的关键。
Objective: To investigate the effect of mechanical ventilation on severe meconium aspiration syndrome (MAS). Methods: Thirty-eight patients with severe MAS were divided into atelectasis group and emphysema group according to the result of blood gas and chest radiograph. The respirator parameters of the two groups were compared and analyzed. Results: Thirty-eight patients were cured in 26 cases (68.4%) and 12 patients died (including abandoned). Common complications were pneumonia, pneumothorax, persistent pulmonary hypertension and neonatal respiratory distress syndrome. Conclusion: According to the different conditions to choose the appropriate ventilator parameters and ventilation mode, active prevention and treatment of complications is the key to the treatment of MAS.
