为了深入探讨慢性粒细胞白血病 (CML)微小残留病 (MRD)生物学特性及病程中MRD在外周血和中枢神经系统中的变化 ,提高CML的检测水平 ,指导临床制订更加科学有效的治疗方案 ,本项目应用套叠式逆转录 /聚合酶链式反应 (RT/PCR)技术对 2 6例CML患者外周血和脑脊液中bcr/ablmRNA进行检测 ,以分析其中白血病细胞 ,发现有 2 4例的外周血和 19例的脑脊液中有融合的bcr/ablmRNA进行了检测 ,以分析其中的白血病细胞 ,发现有 2 4例的外周血和 19例的脑脊液中有融合的bcr/ablmRNA表达 ,另有 4例完全缓解期CML外周血和脑脊液中均有bcr/ablmRNA表达 ,提示该方法是检测MRD ,尤其是中枢神经系统中的MRD ,十分灵敏而又准确的方法 ,对指导临床化疗方案的确定 ,以及预后的提示 ,提高CML的生存率等均有重要价值。
In order to further investigate the biological characteristics of chronic myeloid leukemia (CML) minimal residual disease (MRD) and the changes of MRD in peripheral blood and central nervous system during the course of disease, improve the detection level of CML, and guide the clinical development of more scientific and effective treatment programs, In this project, we use nested reverse transcription polymerase chain reaction (RT/PCR) technology to detect bcr/abl mRNA in peripheral blood and cerebrospinal fluid of 26 CML patients to analyze leukemia cells and found that there are 24 cases of peripheral blood. Blood and 19 cases of bcr/abl mRNA were detected in the cerebrospinal fluid to analyze the leukemia cells. 24 cases of peripheral blood and 19 cases of cerebrospinal fluid were found to have fused bcr/abl mRNA, and 4 cases were found. The expression of bcr/abl mRNA in peripheral blood and cerebrospinal fluid of CML patients during complete remission phase suggests that this method is to detect MRD, especially MRD in the central nervous system. It is a sensitive and accurate method for the determination of clinical chemotherapy regimens and prognosis. The tips for improving the survival rate of CML are of great value.
