犬肋骨膜肌瓣食管成型术的应用解剖及实验研究

目的:为带血供的骨膜肋间肌瓣食管成型术提供解剖学和形态学基础。方法:取10条犬的肋骨膜肌瓣标本,将其上的第7肋间血管灌注后观察分布情况。手术先切除部分第7肋骨,然后将保留的骨膜和上下肋间肌修整成一个带血管蒂的骨膜肌瓣。瓣长85~ 12.5 cm,宽4.5 cm,以此肌瓣修复 1/2直径4cm长的食管缺损。结果:带蒂的骨膜肌瓣由肋间动脉的两条主干分支供血,具有良好的血运。尽管处死犬检查手术修复部位轻度狭窄,但术后实验犬与正常犬在进食与活动上相同。结论:带血供的肋骨膜肌瓣是胸段食管成型的良好组织材料。 OBJECTIVE: To provide an anatomic and morphological basis for the peri-perforated intercostal muscle flap esophageal angioplasty. Methods: Ten dogs’ rib sarcolemmal flap specimens were taken to observe the distribution of the 7th intercostal vascular perfusion. Surgical removal of the first part of the first seven ribs, and then keep the periosteum and upper and lower intercostal muscle trimmed into a periosteal flap with vascular pedicle. Petal length 85 ~ 12.5 cm, width 4.5 cm, the muscle flap repair 1/2 diameter 4cm long esophageal defect. Results: The pedicled periosteal muscle flap was supplied by two main branches of the intercostal artery and had good blood supply. Although the death of dogs to check the site of surgical repair of mild stenosis, but after experimental dogs and normal dogs in eating and activity the same. CONCLUSION: Blood-supplying rib membrane flap is a good tissue material for thoracic esophagectomy.
