目的探讨环氧合酶-2(COX-2)和Ki-67在骨肿瘤中的表达及临床意义。方法应用免疫组化和原位杂交方法检测105例骨肿瘤组织中的COX-2、Ki-67的表达,并对其中85例骨肉瘤主要临床资料、病理分级及临床相关参数进行比较,用χ2检验进行统计学处理。结果骨肉瘤中COX-2、Ki-67的蛋白及mRNA阳性表达率明显高于骨软骨瘤阳性表达率(P<0.01)。COX-2、Ki-67的蛋白及mRNA的阳性表达率在骨肉瘤临床分期中Ⅱb期、Ⅲ期明显高于Ⅰ期、Ⅱa期(P<0.01),并随着病理分级恶性程度增高而显著增加(P<0.01),且在软组织浸润和转移组中COX-2、Ki-67蛋白及mRNA的阳性表达率明显高于非浸润和非转移组(P<0.01)。结论COX-2、Ki-67的蛋白及mRNA在骨肉瘤中不同程度异常表达与肿瘤的临床分期、病理分级和浸润、转移呈正相关,提示COX-2、Ki-67的蛋白及其mRNA高表达尤其在骨肉瘤的发生、发展和浸润转移过程中发挥重要作用。
Objective To investigate the expression and clinical significance of cyclooxygenase-2 (COX-2) and Ki-67 in bone tumors. Methods The expressions of COX-2 and Ki-67 in 105 cases of osteosarcoma were detected by immunohistochemistry and in situ hybridization. The main clinical data, pathological grading and clinical parameters of 85 cases of osteosarcoma were compared with χ2 Test for statistical analysis. Results The positive rates of COX-2 and Ki-67 in osteosarcoma were significantly higher than those in osteochondroma (P <0.01). The positive expression rates of COX-2 and Ki-67 in stage Ⅱb and Ⅲ of clinical stage of osteosarcoma were significantly higher than those in stage Ⅰ and Ⅱa (P <0.01), and were significantly increased with the increase of pathological grade (P <0.01). The positive expression rates of COX-2, Ki-67 protein and mRNA in infiltration and metastasis of soft tissues were significantly higher than those in non-infiltration and non-metastasis groups (P <0.01). Conclusions The abnormal expression of COX-2 and Ki-67 in osteosarcoma is positively correlated with the clinical stage, pathological grade, infiltration and metastasis of tumor, which indicates that the protein and mRNA of COX-2 and Ki-67 are highly expressed Especially in the occurrence, development and invasion and metastasis of osteosarcoma play an important role.
