本文报告自1992年以来采用自行研制的“自锁式”人工椎体治疗14例脊柱肿瘤合并截瘫病例,其中恶性巨细胞瘤2例,软骨肉瘤1例,恶性纤维组织细胞瘤和纤维肉瘤和血管瘤各1例,骨髓瘤4例,转移瘤4例(肾上腺透明细胞瘤、前列腺癌及肺癌)。采取前方八路切除肿瘤减压后,即用人工椎体重建脊柱的稳定性,术后辅以化疗和放疗,结果满意。术后平均随诊了20.6月(3月~3年8月),随诊时仍存活者10例(占71.3%),其中存活了2年以上5例;死亡病例中,术后平均生存19.8月。患者生活质量均得到明显改善,为脊柱肿瘤治疗提供了又一有效手段。
This article reports on the use of self-developed “self-locking” artificial vertebrae to treat 14 cases of spinal tumors with paraplegia since 1992, including 2 cases of malignant giant cell tumors, 1 case of chondrosarcoma, malignant fibrous histiocytoma, and fibrosarcoma and vessels. There were 1 case of tumor, 4 cases of myeloma, and 4 cases of metastatic tumor (adenosic clear cell tumor, prostate cancer, and lung cancer). After the tumor was decompressed and excised in eight directions, the stability of the spine was reconstructed with artificial vertebral body, and chemotherapy and radiotherapy were supplemented with satisfactory results. The average follow-up time was 20.6 months (from March to August 2008), and survivors were still surviving at the time of follow-up (10 cases, 71.3%). Among them, 5 survived for more than 2 years; After the average survival of 19.8 months. The quality of life of patients has been significantly improved, providing another effective means for the treatment of spinal tumors.
