目的探讨结肠癌组织中DPD(二氢嘧啶脱氢酶)、p53(p53基因)蛋白的表达意义及其预测结肠癌患者预后的价值。方法采用免疫组织化学法检测70例结肠癌组织和30例癌旁正常组织中DPD、p53蛋白的表达情况,分析两者在结肠癌中表达相关性,及其与结肠癌患者临床病理特征和5-FU为主化疗疗效、毒性及预后的关系。结果 DPD和p53在癌组织中表达,阳性表达率分别为84.3%和64.3%,均明显高于癌旁正常结肠组织(P<0.05),且两者在结肠癌中呈正相关(r=0.334,P<0.05)。DPD、p53蛋白在结肠癌中表达与肿瘤TNM分期、浸润深度密切相关(P<0.05),与患者年龄(P=0.799)、性别(P=0.617)、肿瘤大小(P=0.215)、分化状态(P=0.859)无关。DPD阳性患者5-FU疗效较阴性患者差(P<0.05),但副作用小(P<0.05)。DPD阳性表达患者生存时间短于阴性表达患者(P<0.05)。结论 DPD、p53与结肠癌的发生发展密切相关,二者的表达强度可以作为患者预后的重要指标。
Objective To investigate the expression of DPD (dihydropyrimidine dehydrogenase) and p53 (p53) protein in colon cancer tissues and predict the prognosis of colon cancer patients. Methods Immunohistochemical method was used to detect the expression of DPD and p53 protein in 70 cases of colon cancer tissues and 30 cases of adjacent normal tissues. The correlation between the expression of DPD and p53 protein in colon cancer was analyzed, and the clinicopathological features of colon cancer patients and 5 -FU is the relationship between efficacy, toxicity and prognosis of primary chemotherapy. Results The expression of DPD and p53 in cancer tissues was 84.3% and 64.3%, respectively, which were significantly higher than those in adjacent normal colon tissues (P<0.05). There was a positive correlation between them in colon cancer (r=0.334, P<0.05). The expression of DPD and p53 protein in colon cancer was closely related to tumor TNM staging and infiltration depth (P<0.05), and was related to patient’s age (P=0.799), gender (P=0.617), tumor size (P=0.215), and differentiation status. (P=0.859) is irrelevant. The efficacy of 5-FU in DPD positive patients was worse than that in negative patients (P<0.05), but the side effects were small (P<0.05). The survival time of patients with DPD positive expression was shorter than that of patients with negative expression (P<0.05). Conclusion DPD and p53 are closely related to the occurrence and development of colon cancer. The expression intensity of both can be used as an important indicator of prognosis.
