目的:探讨良性阵发性位置性眩晕(benign payoxyxmal positmal vertigo BPPV)在听力和前庭功能检查方面的特点。进一步了解BPPV可能发病机制并提出防止性措施。方法:对89例BPPV患者行纯音测听、高刺激率、听性脑干反应、前庭双温试验、前庭诱发的脑源性电位检查。分析:BPPV患者几种耳功能检示阳性率及在BPPV发病机制中的作用。结果:BPPV患者中,原发性BPPV占70.79%。纯音测听异常者占39.33%,脑干反应异常者在原发性BPPV中占60%,前庭功能双温试验异常者占24%,BPPV同侧病变占66.7%,水平半规管病变BPPV占56%。结论:原发性BPPV发病率高于继发性BPPV病变。这一点与国内的钟乃川、张素珍等相同,所不同的是维吾尔族与汉族相比患者纯音测听轻中度感音聋,神经性耳聋以高频的下降为主,水平半规管异常率较高。高频脑干刺激率反应异常对BPPV有一定的影响,这说明迷路病变与BPPV密切相关、内耳供血密切相关。维吾尔族大部分患者由于饮食习惯、动脉硬化、内耳供血直接影响,内耳供血不足易引起耳石脱落,实行耳石复住的同时要用改善循环的药物。
Objective: To investigate the characteristics of benign payoxyxmal positmal vertigo (BPPV) in hearing and vestibular function examination. Learn more about the possible pathogenesis of BPPV and propose preventive measures. Conclusion: The incidence of primary BPPV is higher than that of secondary BPPV. This is the same as Zhong Nai Chuan, Zhang Suzhen and other domestic, the difference is the Uygur and Han patients compared to pure tone audiometry mild to moderate deafness, neurological deafness to high-frequency decline, the higher the level of abnormal semicircular canal . Abnormal response to high frequency brainstem stimulation of BPPV have a certain impact, indicating that lost lesions and BPPV are closely related to the inner ear blood are closely related.
