目的:1.探讨免疫分型对成人急性白血病(AL)的诊断价值;2.分析成人急性白血病免疫表型特点及免疫亚型分布情况。方法:采用流式细胞仪结合单克隆抗体双色直接免疫荧光标记方法检测75例急性白血病免疫表型;用瑞氏染色及细胞化学判断骨髓形态学结果。结果:1.75例AL中6例形态学与免疫分型不相符占8%。2.ANLL主要表达CyMPO和CD33,ALL主要表达CD79a、CD20。3.AML组中CyMPO阳性表达率远高于ALL组,差异显著(P<0.01),CDll7在Ly+-AML组中阳性表达率远高于Ly--AML组(P<0.01)。结论:1.白血病免疫分型是对形态学分型的补充和修正,对于B-ALL、T-ALL、Mo及HAL诊断必可少。2.MPO是髓系特异标志,CDll7与Ly+-AML发生密切相关。
Objective: 1. To investigate the diagnostic value of immunophenotyping in adult acute leukemia (AL); 2. To analyze the immunophenotypic characteristics and immune subtype distribution of adult acute leukemia. Methods: The immunophenotypes of 75 cases of acute leukemia were detected by flow cytometry combined with monoclonal antibody direct immunofluorescence staining. The bone marrow morphology was determined by Wright staining and cytochemistry. Results: The morphology and immunophenotype of 6 cases in 1.75 cases of AL were not consistent with 8%. 2.ANLL mainly expresses CyMPO and CD33, and ALL mainly expresses CD79a and CD20.3.The positive expression rate of CyMPO in AML group is much higher than that in ALL group (P <0.01), the positive expression rate of CD117 in Ly + -AML group is far Higher than Ly - AML group (P <0.01). Conclusion: 1. Leukemia immunophenotyping is the supplement and revision of morphological classification, for the B-ALL, T-ALL, Mo and HAL diagnosis less. 2.MPO is a myeloid specific marker, CDll7 and Ly + -AML closely related.
