40例活动性十二指肠球部溃疡(DU)病人均伴有胃窦炎,28例胃窦部幽门螺旋菌(HP)阳性,12例阴性。基础胃酸分泌(BAO)、五肽胃泌索刺激后的高峰胃酸分泌(PAO)以及空腹和餐后1小时血清胃泌素水平,在HP(+)组均明显高于HP(-)组(P<0.01~0.02)。经治疗,10例HP(+)患者复查,发现HP清除后空腹和餐后血清胃泌素均显著下降(P<0.01),但平均BAO和PAO下降不明显(P>0.05)。研究结果提示,HP系通过刺激胃泌素分泌及胃酸增高而引起DU,HP清除后血清胃泌索下降,PAO无明显下降。鉴于此,采用杀灭HP药物结合H2受体阻断剂治疗,可望加速溃疡愈合。
Forty patients with active duodenal ulcer (DU) were accompanied by antral gastritis, and 28 cases of gastric antrum were positive for Helicobacter pylori (HP), 12 were negative. Basal gastric acid secretion (BAO), peak gastric acid secretion (PAO) after pentagastrin stimulation and fasting and postprandial one hour serum gastrin levels in HP (+) group were significantly higher than those in HP (-) group P <0.01 ~ 0.02). After treatment, 10 cases of HP (+) patients were reviewed, fasting serum and postprandial serum gastrin were significantly decreased (P <0.01), but the average BAO and PAO decreased not significantly (P> 0.05) . The results suggest that, HP Department through the stimulation of gastrin secretion and increased gastric acid caused by DU, HP after removal of serum gastrin decreased, PAO no significant decline. In view of this, the use of anti-HP drugs combined with H2 receptor blocker therapy, is expected to accelerate ulcer healing.
