目的研究MBL、MASP和C3b在不同感染程度的乙肝患者中的表达及其临床意义。方法乙肝患者共140例,依据血清中HBeAg和ALT的水平分为6组:A组(HBeAg阳性,ALT正常)、B组(HBeAg阴性,ALT正常)、C组(HBeAg阳性,ALT轻度/中度升高)、D组(HBeAg阳性,ALT高度/重度升高)、E组(HBeAg阴性,ALT轻度/中度升高)、F组(HBeAg阴性,ALT高度/重度升高)。采用ELISA法检测健康对照(G组)及患者血清中MBL、MASP和C3b的水平,并对这三者之间的水平做相关性分析。结果 A组的MBL水平显著高于C组和D组(P<0.05),G组的MBL水平高于除B组外的其余5组(P<0.05)。F组的MASP水平显著高于A、C、D组,同时其显著低于B、G组(P<0.05),D组的MASP水平显著低于其余6组(P<0.05),G组的MASP水平显著高于其余6组(P<0.05)。D组和F组的C3b水平显著高于A、B、C、E组(P<0.05),但D、F组之间并无显著差异(P>0.05),G组的C3b水平显著低于其余6组(P<0.05),MBL与MASP之间存在中度正相关性,而C3b与MASP、MBL之间存在弱的负相关性(P<0.05)。结论 MBL与机体清除HBV有关,高水平C3b可能会提高乙肝病变程度,MASP水平异常降低与乙肝患者肝损伤程度加重、病毒感染和复制水平加剧均有关系。
Objective To study the expression and clinical significance of MBL, MASP and C3b in hepatitis B patients with different infection levels. Methods A total of 140 patients with hepatitis B were divided into 6 groups according to the levels of serum HBeAg and ALT: group A (HBeAg positive, normal ALT), group B (HBeAg negative, normal ALT), group C (HBeAg positive, ALT mild / Moderate increase), Group D (HBeAg positive, ALT / severe increase), Group E (HBeAg negative, ALT mild / moderate increase) and Group F (HBeAg negative, ALT / severe increase). The levels of MBL, MASP and C3b in serum of healthy controls (G group) and patients were detected by ELISA, and the correlation between these three levels was analyzed. Results The MBL level in group A was significantly higher than that in group C and group D (P <0.05). The MBL level in group G was higher than that of group B except the other 5 groups (P <0.05). The level of MASP in group F was significantly higher than that in group A, C and D, and significantly lower than those in group B and G (P <0.05). The level of MASP in group D was significantly lower than that in the other six groups (P <0.05) MASP levels were significantly higher than the other 6 groups (P <0.05). The level of C3b in group D and group F was significantly higher than that in group A, B, C and E (P <0.05), but there was no significant difference between group D and F (P> 0.05) There was a moderate positive correlation between MBL and MASP in the other 6 groups (P <0.05), but a weak negative correlation between C3b and MASP and MBL (P <0.05). Conclusions MBL is related to the clearance of HBV by the body. High level of C3b may increase the severity of hepatitis B. The abnormal decrease of MASP is related to the increased degree of liver injury and the increase of viral infection and replication level.
