粪石多发生在胃内。食管石是极罕见的,常发生在具有食管运动障碍和解剖异常的老年人。本文作者报导了一例发生在没有食管异常的年轻病人的食管石。病人为一27岁既往健康的男性,由于剧烈呕吐后发生进行性吞咽困难和胸痛一周而入院。钡餐检查发现食管中下三分之一交界处有一不规则腔内肿块,胃窦小弯有一溃疡。手术见食管内肿块为粪石。以前认为食管石多发生在有食管解剖异常如憩室和狭窄以及食管运动障碍的老年人。本例为一年轻人,既没有食管狭窄也没有食管憩室,唯一发现是胃溃疡。而且病人在剧烈呕吐后发生严重的进行性吞咽困难,说明呕吐过程中胃石被带到食管中去,没有被吐出而存留于食管内。
Stones occur in the stomach more. Esophageal stone is extremely rare and often occurs in the elderly with esophageal dyskinesia and anatomical abnormalities. The authors report an esophageal stone that occurred in young patients without esophageal anomalies. The patient was a healthy, 27-year-old man admitted to hospital for a week of progressive dysphagia and chest pain after severe vomiting. Barium meal examination found that the lower third of the esophagus at the junction of an irregular cavity mass, a small ulcer in the antrum. Surgical see esophageal mass is dung stone. In the past that esophageal stone occurred in the esophageal anatomy abnormalities such as diverticulosis and stenosis and esophageal dyskinesia in the elderly. This case is a young man, neither esophageal stricture nor esophageal diverticulum, the only discovery is gastric ulcer. And the patient developed severe progressive dysphagia after vigorous vomiting, indicating that gastritis was taken into the esophagus during vomiting and was not spit out and remained in the esophagus.
