【目的】探讨“双 C”疗法联合维生素 D 治疗高龄妊娠期糖尿病(GDM)患者的疗效。【方法】选择高龄GDM 患者110例,采用随机数表法分为两组,每组55例,对照组采用传统多次胰岛素皮下注射,试验组则采用“双 C”疗法联合应用维生素 D 治疗。比较两组患者治疗两周后的血糖控制、妊娠和新生儿情况。【结果】试验组空腹血糖(FPG)为(6.09±0.76)mmol/L、餐后2 h 血糖(2hPG)为(8.99±1.13)mmol/L、血糖波动幅度为(5.74±1.38)mmol/L、胰岛素平均用量(0.54±0.09)U/(kg·d)和糖化血红蛋白(HbAlc)含量(7.52±0.75)%,均显著低于对照组,血糖达标时间更短,仅为(4.46±1.37)d,其差异均具有统计学意义(P <0.05)。试验组患者低血糖、妊娠期高血压疾病、胎膜早破、羊水过多、新生儿死胎、新生儿窒息、早产儿的发生率较对照组明显降低(P 0.05)。【结论】“双 C”治疗方案联合维生素 D 治疗高龄妊娠期糖尿病可更快速平稳的控制血糖,降低母婴并发症及产后低血糖的发生率,提高新生儿预后,是一种有效、安全的治疗方案。“,”Objective]To investigate the efficacy and safety of the combination of“Double C”and Vitamin D in the treatment of elderly patients with gestational diabetes mellitus.[Methods]110 cases of elderly patients with gestational di-abetes mellitus were selected.They were divided into control group and experimental group randomly with random indica-tor method.Each group had 55 cases,the control group was given the traditional multiple subcutaneous insulin injection, the experimental group was given the“double C”therapy in combination with vitamin D treatment so as to compare the sugar blood control,pregnancy and newborn of the two groups two weeks after treatment.[Results]The fasting blood glucose of the experimental group (FBG)was (6.09 ±0.76)mmol/L,the postprandial 2h blood glucose was (8.99 ± 1.13)mmol/L,the blood glucose fluctuation range was (5.74 ±1.38)mmol/L,the average insulin dosage (0.54±0.09) U/(kg·d)and the glycosylated hemoglobin content (7.52 ± 0.75)% ,which were significantly lower than those in the control group .The time for blood sugar to reach the standard was shorter,only (4.46±1.37)d,and the difference was statistically significant (P <0.05).Compared with the control group,the occurrence rate of hypoglycemia,gestational hypertension disease,premature rupture of membranes,polyhydramnios,stillbirth,neonatal asphyxia and premature of the patients in the experimental group decreased significantly (P 0.05).[Conclusion]“Double C”treatment with vitamin D therapy for elderly gestational diabetes can control blood sugar more quickly and smoothly,reduce maternal and infant complications and the incidence of postpartum hypoglycemia ,and improve the prognosis of neonatal.Therefore,it is an effective and safe treatment.
