目的了解江苏高邮农村代谢综合征(MS)患病分布情况。方法 2009-12-2010-05,对江苏高邮农村18~74岁汉族居民进行抽样调查,包括标准化的问卷调查,测量血压、腰围、臀围及体质量指数、腰臀比等体格指标并采血进行血糖、血脂等生化指标检验,共对4493份资料进行分析。对入选居民分别根据国际糖尿病联盟(IDF)推荐的有关中国人MS的诊断标准和美国胆固醇教育计划成人教育组Ⅲ(NCEP-ATPⅢ)亚洲人群修订标准诊断MS,计算MS患病率,采用多变量logistic回归分析MS患病率相关因素。结果江苏高邮农村18~74岁汉族居民,MS患病粗率:IDF标准为27.0%(男性为20.5%,女性为32.7%,P<0.01),NCEP-ATPⅢ标准为16.2%(男性为15.6%,女性为16.8%,P=0.296);根据中国人口年龄进行标化后,MS患病率:IDF标准为16.3%(男性为16.0%,女性为18.0%);NCEP-ATPⅢ标准为9.4%(男性为11.0%,女性为9.1%)。两种诊断标准有较好的符合率(87.7%)。45岁以前,MS患病率随年龄增加而升高,男性高于女性,差异有统计学意义(P<0.01);45岁以后,MS患病率女性随年龄增加而升高,而男性随年龄增加而降低,女性高于男性,差异有统计学意义(P<0.01)。Logistic回归分析提示MS患病率与性别、糖尿病家族史、高血压家族史、教育程度、体力活动、脉压、静息心率、糖化血红蛋白、血尿酸等因素相关。结论江苏高邮农村18~74岁汉族居民MS患病率略高于国内其他地区。应采取综合措施降低MS发生率。IDF标准和NCEP-ATPⅢ诊断符合率较好,但IDF标准更适用于中国人群。
Objective To understand the prevalence of metabolic syndrome (MS) in rural Gaoyou in Jiangsu Province. Methods A sample survey of Han residents aged 18-74 years in rural Gaoyou, Jiangsu Province, from 2009 to 20 of 2009 was conducted on a standardized questionnaire to measure physical parameters such as blood pressure, waist circumference, hip circumference, body mass index and waist-to-hip ratio Blood glucose, blood lipids and other biochemical tests, a total of 4493 data analysis. Residents were selected to diagnose MS according to the diagnostic criteria of Chinese MSD recommended by the International Diabetes Federation (IDF) and the Asian population of the National Cholesterol Education Program Adult Education Group Ⅲ (NCEP-ATPⅢ) respectively. The prevalence of MS was calculated using multivariate Logistic regression analysis of the prevalence of MS related factors. RESULTS: The prevalence of MS in Han residents aged 18-74 years in Gaoyou, Jiangsu Province was 27.0% for ID patients (20.5% for males and 32.7% for females, P <0.01) and 16.2% for NCEP-ATPⅢ patients (15.6% The prevalence of MS was 16.3% (16.0% for males and 18.0% for females); the NCEP-ATPⅢ was 9.4% (males for 16.8% of females, P = 0.296) 11.0% and women 9.1%). The two diagnostic criteria have a good coincidence rate (87.7%). Before the age of 45, the prevalence of MS increased with age, and was higher in males than in females (P <0.01). After age 45, the prevalence of MS increased with age in males The age was lower and the female was higher than the male, the difference was statistically significant (P <0.01). Logistic regression analysis showed that the prevalence of MS was related to gender, family history of diabetes, family history of hypertension, educational level, physical activity, pulse pressure, resting heart rate, glycosylated hemoglobin and serum uric acid. Conclusion The prevalence of MS in Han residents aged 18-74 years old in Gaoyou rural area in Jiangsu Province was slightly higher than that in other areas in China. Should take comprehensive measures to reduce the incidence of MS. IDF standard and NCEP-ATP Ⅲ diagnosis coincidence rate is better, but the IDF standard is more suitable for the Chinese population.
