[目的]分析报告郑州市2005~2007年志贺菌菌型和耐药性鉴定结果,为指导临床用药提供依据。[方法]志贺菌监测、分离、鉴定依照《全国细菌性痢疾监测方案》,采用NCCLS推荐的K-B琼脂纸片扩散法检测耐药性。[结果]在检出的116株志贺菌属中,福氏志贺菌97株,占83.6%,其中F4c菌47株(40.5%),F2a菌17株(14.7%),F1a菌21株(18.1%),Fx变种4株(3.4%),F2b菌5株(4.3%),Fy菌1株(0.9%),F4a菌2株(1.7%),宋内志贺菌19株(16.4%),未检出A群和C群;志贺菌对萘啶酸和四环素100%耐药,对氨苄西林、利福平、复方新诺明和氯霉素,耐药率分别为94.2%、88.4%、79.7%、75.4%。[结论]F4c菌为郑州市志贺菌流行的优势菌株,多重耐药现象严重,达到100%,临床治疗应在药敏试验指导下进行。
[Objective] To analyze and report the results of identification of Shigella strains and drug resistance in Zhengzhou from 2005 to 2007, and provide the basis for guiding clinical drug use. [Method] Shigella monitoring, isolation and identification According to the “National Bacterial dysentery monitoring program”, KCC agar disk diffusion method recommended by NCCLS was used to detect drug resistance. [Result] Among 116 strains of Shigella detected, 97 strains of Shigella flexneri were 83.6%, of which 47 strains were F4c (40.5%), 17 strains were F2a (14.7%), 21 strains were F1a (18.1%), Fx strain 4 (3.4%), F2b strain 5 (4.3%), Fy strain 1 (0.9%), F4a strain 2 (1.7%), Shigella sonnei 19 %). A group and C group were not detected. Shigella was 100% resistant to nalidixic acid and tetracycline, the resistance rates to ampicillin, rifampicin, cotrimoxazole and chloramphenicol were 94.2% 88.4%, 79.7%, 75.4%. [Conclusion] F4c strain is the predominant strain of Shigella flexneri which is endemic to Zhengzhou City. The multi-drug resistance is serious, reaching 100%. Clinical treatment should be conducted under the guidance of drug sensitivity test.
